As the designated CDDO for Ford, Ness, Hodgeman, Gray, Meade, Clark, Edwards, Kiowa, Comanche, Pratt, Barber, Kingman, and Harper Counties Arrowhead West has adopted the following procedures:

Establish Written Procedures
AWI as the CDDO, hereby establishes written procedures which specify how the requirements of K.A.R. 30-64-01 are met.

Single Point of Application and Referral
AWI shall be the single place where persons apply for developmental disabilities services in Ford, Edwards, Ness, Meade, Gray, Comanche, Hodgeman, Clark, Kiowa, Barber, Harper, Kingman, and Pratt counties of Kansas.

Statewide Waiting List
AWI will administer and manage a waiting list for community services. AWI is the single point of application and referral for persons wanting community services.

Gate-keeping
AWI will review persons living in it's service area who apply for admission to private or public ICFs/MR (Public ICFs/MR are the state Mental Retardation Hospitals).

The Council of Community Members
AWI has adopted a separate set of procedures for a Community Council. The procedures are available for review to interested parties.

Dispute Resolution
AWI's procedures for Dispute Resolution are included in the procedures of the Community Council.

Quality Assurance
AWI's procedures for Quality Assurance are included in the procedures for the Community Council. Members of the Community Council and other person's served will also participate in AWI's quarterly Safety Committee process.

Quality Enhancement
AWI's procedures for Quality Enhancement are included in the Community Council procedures and through the quarterly Case Review/Program and Quality Assessment Committee.

Case Management
Case Management is a service. The person and the person's guardian (if applicable), will choose who, from among qualified enrolled Case Managers, provides Case Mangement to them. The person providing Case Management must meet the minimum requirements of this service and to provide the service(s) requested. Each person may have only one Case Manager who bills for Case Management on the person's behalf. The Case Manager must accept full responsibility to provide all the components of Case Management in any and all environments needed. A person may choose not to receive Case Management as long as the person or the person's guardian does not insist Case Management functions are carried out on behalf of AWI or it's affiliates.

Uniform Access to Services
Neither AWI nor any of it's affiliate providers may discriminate in selecting persons to serve based on the persons' severity of disability. Only the Secretary of SRS or her designee may determine if a person is inappropriate for community services. This determination may be made by the Secretary or her designee only if the person presents a clear and present danger to self or the community even when provided appropriate community services.

Continuity and Portability
Each person who has begun to receive services must be allowed to move from one CDDO service area to another or from one community service provider to another.
